1) Name
2) Address (include city)
3) Daytime phone
4) Work you're requesting
A) Spring startup and adjustment - $65
B) Annual backflow certification - $75
C) Winterization - $75
D) Additional requests (ie. request for repairs, estimates on additional irrigation needs, etc)
Receipt of an email request will serve as a formal request for services.
I will reply within 24hr indicating the date when you should expect the work to be completed.

5) Contact infomation to set up an appointment - indicate email address to receive a return email or daytime phone number if a callback is desired.
